Healthy Lives Advisor children and young people

Social Prescribing for children and young people aged 11- 18 years.

Children and young people’s Social Prescribing is a non-medical approach to improving your health and wellbeing. It can help you to have more control over your own health and find ways to improve how you feel in a way that suits you; The Healthy Lives Advisor for children and young people will work together with you to develop an action plan around what matters to you.  Children and young people might be referred to the Health Lives Advisor for a range of issues including:

 

  • Children and young people who may feel isolated and want support to meet other people
  • Children and young people who may have a long-term chronic health condition which would be helped through community activities
  • Children and young people who may need help to find support or advice with practical issues such as money or housing
  • Children and young people who may want help and support with their wellbeing and education.

Social Prescribing is not designed to replace medical support but getting involved with local groups and activities can help you get better and feel better faster than medicine alone. Your Healthy Lives Advisor might introduce you to a community group or a new activity. They may help you to meet new people or, they might help you find information or access advice about a particular issue.

A referral can be made to the Healthy Lives Advisor by your GP.

One Health and Care – NOW LIVE

Did you know? From summer 2021, everyone’s health and social care records in Shopshire, Telford and Wrekin will be available on the new integrated care record ‘One Health and Care’.

Completely confidential and secure, it’s designed to help doctors, nurses and other registered health and social care professionals directly involved in your care to make better, safer decisions.

The information that they will be able to see includes:

  • Your name, date of birth, sex, address, telephone number, NHS number
  • The name of your GP Practice and GP
  • Medications, allergies, ongoing and historic conditions, immunisations and diagnoses
  • Test results, hospital referrals, admissions, discharges and clinics attended
  • Social and mental health information and care plans

Sensitive information such as attendance at sexual health clinics, fertility treatment records, and records relating to gender reassignment will not be included.

During summer 2021, all GP practices within Shopshire, Telford and Wrekin will have access to One Health and Care along with the following organisations:

  • Shropshire Community Health NHS Trust
  • Midlands Partnership NHS Foundation Trust
  • Shropshire Council (Social care)
  • Telford & Wrekin Council (Social care)
  • Shrewsbury and Telford Hospital NHS Trust
  • Robert Jones & Agnes Hunt Orthopaedic Hospital NHS Foundation Trust

 

If you’re happy for your records to be available on One Health and Care to health and social care professionals, you don’t need to do anything.  Alternatively, if you wish to object, you need to contact your GP Practice.

GP Winter Pressures Extended Opening Hours Clinics

We would like all patients in our community to be able to see a local GP or Nurse when they need to. We have set up a network of GP Practices across the region offering extended opening hours for pre-bookable and same day routine extended access appointments. The GP extended opening hours service operates between 6.00 pm - 8.00 pm on weekdays and 8.00 am-1.00 pm on weekends and allows patients to be seen within a network of GP Practices at a convenient time and location, depending on availability.

What is an NHS Health Check?

The NHS Health Check is a health check-up for adults in England aged 40-74. It's designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.

Specialists hit the streets to help tackle men’s health issues “Don’t be embarrassed” is the message from GPs, pharmacists and healthcare professionals across Shropshire, Telford and Wrekin. From Monday 7 to Saturday 12 November, the local NHS, alongside partner organisations, is hosting its own ‘Men’s Health Week’ to tie in with national ‘Movember’ charity work. The local campaign aims to raise awareness of key health issues such as prostate cancer symptoms, abdominal aortic aneurysm screening and the prevalence of hidden mental health concerns. Local GP and Clinical Lead for South-East Shropshire, Dr Jess Harvey, said: “We decided to hold a campaign across the county to highlight the importance of men’s health.“We are holding four pop-up events throughout the week, and will be visiting local shopping centres and football matches. We’re really looking forward to chatting with passers-by and giving advice where needed. “GPs will be joined by pharmacists, mental health therapists, screening specialists and local cancer charity Lingen Davies. You can pop along to get your Covid autumn booster too.” The events will run on: 

  • Wednesday 9 November at Darwin Shopping Centre, Shrewsbury, SY1 1PL 10am-4pm
  • Thursday 10 November at Telford Shopping Centre, Telford TF3 4BX 10am-4pm
  • Saturday 12 November at AFC Telford match Wellington, TF1 2TU 12-3pm
  • Saturday 12 November at Shrewsbury Town FC match SY2 6ST 12-3pm

 Dr Nick White, Chief Medical Officer at NHS Shropshire, Telford and Wrekin said: “We know men can sometimes be embarrassed of physical symptoms, but this is what we are trained for. “We are urging men across the county to come forward and book an appointment with a GP if something doesn’t feel right – maybe you’re going to the toilet too often, have found a lump or are feeling down. We’re here to help.“I’m sure we all know someone who is experiencing a worrying symptom and yet doesn’t feel comfortable seeking medical help. My simple message is this – there is no need to be embarrassed, we have seen it all, so please get an appointment booked with a doctor.”

Extended Access – Local GP practices working together to offer more convenient appointment times for patients

 

As from 1st October, 2018, we will be part of a network of practices (known as ‘hubs’) in Shropshire, working together to offer patients ‘Extended Access’ to pre-bookable appointments - in the evenings, at the weekend and during bank holidays.

 

Part of the national agenda by NHS England, Extended Access means patients will be able to make pre-bookable appointments to see a GP, practice nurse, or other qualified healthcare professional, at a time which may be more convenient for them.

 

In Shropshire, local practices will work together in a network of ‘hubs’ to provide this service. The ‘hubs’ will take turns to provide the Extended Access appointments to patients. This means patients could be offered an appointment at their own surgery or at a nearby practice in the local area (part of the hub). Patients will be advised about appointment location at time of booking, enabling them to make an informed choice about their where and when they access care.

 

Locally the pre-bookable appointments will be available as follows:

    • Weekday evenings (Monday to Friday) between 6.30pm and 8pm (at a local area hub)

    • Saturdays between 8am and 1pm (at a local area hub)

  • Sundays and bank holidays between 8.30am and 12.30pm (at a single site in central Shrewsbury)

 

Arranging a pre-bookable evening, weekend or bank holiday appointment is easy to do - patients simply need to contact us, during normal opening hours, and speak to the practice receptionist or a member of the practice team who will be able to help. Appointments may not be with your registered GP but will be with a GP or other qualified healthcare professional.

 

Extended Access adds to the existing range of healthcare services already available in Shropshire to help patients, including pharmacists, NHS 111, the walk-in centre, minor injury units, and A&E (which is there 24/7 for life threatening serious emergencies only).

It is important to be aware of how long common conditions should take to clear up

and always seek medical advice if you are unsure or if you feel it is an emergency.

 

Extended Access comes into effect as from 1st October, 2018.
