GP Winter Pressures Extended Opening Hours Clinics

We would like all patients in our community to be able to see a local GP or Nurse when they need to. We have set up a network of GP Practices across the region offering extended opening hours for pre-bookable and same day routine extended access appointments. The GP extended opening hours service operates between 6.00 pm - 8.00 pm on weekdays and 8.00 am-1.00 pm on weekends and allows patients to be seen within a network of GP Practices at a convenient time and location, depending on availability.

Our practice is taking part in an important national project about Diabetes care and treatment in the NHS. The project is called the National Diabetes Audit (NDA.

To take part we will be sharing information about your diabetes care and treatment with the NDA, the type of information, and how it is shared, is controlled by law and enforced by strict rules of confidentiality and security.  For further infrmation about how your information is used please see the NDA patient information leaflet.

Taking part in the NDA shows that this GP practice is committed to improving care for people with Diabetes.

If you do not want your information to be used, please inform the receptionist, your GP or Nurse. This will not affect your care. More information is available - please ask a member of staff for a patient information leaflet.

What is an NHS Health Check?

The NHS Health Check is a health check-up for adults in England aged 40-74. It's designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.

Home Visits

Doctor's bagWhilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling reception between 08:30 - 11:00.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls

You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
