Healthy Lives Advisor children and young people

Social Prescribing for children and young people aged 11- 18 years.

Children and young people’s Social Prescribing is a non-medical approach to improving your health and wellbeing. It can help you to have more control over your own health and find ways to improve how you feel in a way that suits you; The Healthy Lives Advisor for children and young people will work together with you to develop an action plan around what matters to you. Children and young people might be referred to the Health Lives Advisor for a range of issues including:

Children and young people who may feel isolated and want support to meet other people
Children and young people who may have a long-term chronic health condition which would be helped through community activities
Children and young people who may need help to find support or advice with practical issues such as money or housing
Children and young people who may want help and support with their wellbeing and education.

Social Prescribing is not designed to replace medical support but getting involved with local groups and activities can help you get better and feel better faster than medicine alone. Your Healthy Lives Advisor might introduce you to a community group or a new activity. They may help you to meet new people or, they might help you find information or access advice about a particular issue.

A referral can be made to the Healthy Lives Advisor by your GP.

As a healthcare environment will still expect patients and staff to wear a mask in the surgery to ensure we protect the vulnerable.

One Health and Care – NOW LIVE

Did you know? From summer 2021, everyone’s health and social care records in Shopshire, Telford and Wrekin will be available on the new integrated care record ‘One Health and Care’.

Completely confidential and secure, it’s designed to help doctors, nurses and other registered health and social care professionals directly involved in your care to make better, safer decisions.

The information that they will be able to see includes:

Your name, date of birth, sex, address, telephone number, NHS number
The name of your GP Practice and GP
Medications, allergies, ongoing and historic conditions, immunisations and diagnoses
Test results, hospital referrals, admissions, discharges and clinics attended
Social and mental health information and care plans

Sensitive information such as attendance at sexual health clinics, fertility treatment records, and records relating to gender reassignment will not be included.

During summer 2021, all GP practices within Shopshire, Telford and Wrekin will have access to One Health and Care along with the following organisations:

Midlands Partnership NHS Foundation Trust
Shropshire Council (Social care)
Telford & Wrekin Council (Social care)
Shrewsbury and Telford Hospital NHS Trust

If you’re happy for your records to be available on One Health and Care to health and social care professionals, you don’t need to do anything. Alternatively, if you wish to object, you need to contact your GP Practice.

GP Winter Pressures Extended Opening Hours Clinics

We would like all patients in our community to be able to see a local GP or Nurse when they need to. We have set up a network of GP Practices across the region offering extended opening hours for pre-bookable and same day routine extended access appointments. The GP extended opening hours service operates between 6.00 pm - 8.00 pm on weekdays and 8.00 am-1.00 pm on weekends and allows patients to be seen within a network of GP Practices at a convenient time and location, depending on availability.

What is an NHS Health Check?

The NHS Health Check is a health check-up for adults in England aged 40-74. It's designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.
